Do morning care; assist in grooming and personal hygiene of patients like shampooing, TSB or bed bathing, oral care, perineal care, and toileting. Do bed making. Assist in mobilization, walking, transferring, and other activities of daily living. Assist the nurse in medication administration. Help the nurse in monitoring/recording vital signs and intake – output; sterile wound dressing and irrigation; in making sure that bodily tubes are aseptically taken cared properly; in applying liniments, creams, and ointments; and in preparing the instruments to be used for any diagnostic and therapeutic procedures. Alleviate incontinence problems of elderly and disabled patients. Prevent bedsores and contractures especially the bedridden.  Assure that hygienically prepared meals are served on time and assist the patients in eating or feeding them as necessary. Educate patients & relatives on relaxation and energy saving techniques, progressive self-care activities, and appropriate food selection to maintain optimum nutrition necessary to maintain a balanced diet lifestyle. Respond to patients’ calls either from their signal light or bell and advice patients of their incoming messages. Give support to patients needing psychological, emotional and spiritual help. Provide comfort measures to patients like backrub and light massage. Convey patients using wheelchair, stretcher, and other modes of patients’ transfer. Escort patients to doctors’ offices and diagnostic/therapeutic procedures.  Participating in the assessment, planning, deliberation, and implementation of the Individual Nursing Care Plan of patients. Endorse properly the patients & the ward to the incoming staff. Advise supervisors of any untoward manifestations, observations, and complaints of patients. Document pertinent data gathered on patient. Assure confidentiality of patient record. Properly collect, label, & bring specimen (like urine, stool, bodily discharge) to the laboratory.  Maintain a clean, orderly, restful and therapeutic environment. Implement the hospital protocols, policies, rules, and regulations. Keep good working relationship with the health team, patients, & their relatives.  Do postmortem care.
